Mission Statement:
St. Joseph's Continuing Care Centre of Sudbury aims to provide specialized care and services to adults requiring rehabilitative, supportive, or transitional care. The goal is to maximize patients' functional potential and help them transition back to independent living, whether at home, a retirement residence, or an assisted living environment.

Geographic Focus:
The organization operates in Sudbury and the surrounding area. It serves adults in North East Ontario, including Algoma, Cochrane, Sudbury–Manitoulin–Parry Sound, Nipissing–Temiskaming, and James and Hudson Bay Coast.

Program Highlights:
- Rehabilitative Care: Offers time-limited, coordinated interprofessional rehabilitation plans at a low intensity. This includes participation in daily functional activities, exercise programs, and therapy to restore independence for basic care needs.
- Complex Medical Management: Provides medically complex and specialized services over an extended period to maintain or slow the rate of functional loss. An interprofessional team works with patients and their families to develop goals supporting quality of life and maximum functional ability.
- Recreational Programs: The Recreation Department offers various on-floor programs such as ceramics, baking, daily mass, arts and crafts, manicures, poetry reading, and pet therapy to enhance patients' quality of life.
