Hornby Denman Health (also known as Hornby and Denman Community Health Care Society) strives to improve the health and wellbeing of Denman and Hornby Islanders. Their mission is to optimize health and health services for residents through advocacy and providing community-based support and services. Their vision is "Healthy Islanders" and they value integrated, community-based, compassionate, prevention-based, and inclusive approaches to healthcare. The organization began in 1978 when volunteers documented over a thousand hours of nursing service provided to the community.

The organization focuses exclusively on serving the residents of Hornby and Denman Islands in British Columbia. These are remote rural northern Gulf Islands where the society provides local, community-based programs and services. They maintain offices on both islands, with their main office on Hornby Island and a second location at the Denman Health Centre. They acknowledge that they operate on the ancestral, traditional, and unceded gathering places of the K'omoks First Nation.

Hornby Denman Health offers a wide range of programs that support islanders of all ages. Their services include home care and home assist, seniors day programs, volunteer transportation, and medical equipment loans. They provide mental health support through child, youth and family counselling as well as adult mental health and substance use services. The organization also coordinates the Hornby Hamper program, which provides food hampers during holidays to families and individuals in need. They host monthly network meetings to bring together local health and community organizations for coordinated service delivery.
