Mission Statement
The Association of Ontario Health Centres, now called the Alliance for Healthier Communities, is dedicated to advancing health equity across Ontario. The organization’s purpose is to ensure that everyone can reach their best possible health, regardless of any barriers they might face. Its members are united in promoting comprehensive primary health care as a fundamental human right, especially for individuals and communities who experience health disparities due to social, economic, or environmental challenges.

Geographic Focus
The Alliance for Healthier Communities focuses its efforts throughout the province of Ontario. It represents more than 100 community-governed primary health care organizations, including Community Health Centres, Aboriginal Health Access Centres, Community Family Health Teams, and Nurse Practitioner-Led Clinics. These organizations serve communities in major city centres and rural areas across Ontario, striving to address health needs province-wide.

Program Highlights
The Alliance and its members deliver a wide range of programs tailored to the diverse needs of Ontario’s communities. They promote community health and well-being, advocate for public health policy improvements, and support primary health care organizations to tackle social determinants of health. Their work includes supporting and coordinating member health centres, conducting research on health equity, and developing strategies to foster healthier, more inclusive communities in Ontario. They also lead and participate in research to make the health care system more sustainable and equitable.
