The Problem

Addiction recovery systems are fragmented, difficult to navigate, and slow to respond.

People seeking recovery from substance use disorders face a fragmented and siloed system of care. Disconnected services, providers, and resources—along with long waits and complex referral processes, often shaped by rigid systems and limited coordination—make it difficult for people to access addiction recovery and related supports when they need them most.

Rethinking how addiction recovery is delivered creates an opportunity to move beyond fragmented systems. By intentionally coordinating the work of existing community-based organizations and aligning recovery with comprehensive supports such as mental health, housing, and other essential services, care can be delivered more efficiently and seamlessly—personalized to each person’s recovery journey. When funding is designed to follow individuals rather than institutions, resources can be deployed where and when they are needed most.

Our Solution

•Technology-enabled, participant-led funding: We invest in the technology and infrastructure that make participant-led funding possible. This model allows resources to follow individuals through their recovery journey, directing funding to the services they actually use rather than relying on block funding models that can leave capacity underused. The result is a more efficient, responsive, and person-centred way to support recovery.

•Recovery & wrap-around services: We support addiction recovery services and the wrap-around supports people need to sustain recovery, including aftercare services, mental health care, housing, wellness, and others. Recovery is not one-size-fits-all – it must be tailored to each individual’s unique needs, and care must extend beyond treatment alone.

•A coordinated network of care: We invest in the coordination and shared infrastructure that connect community-based organizations into a strong network of care. Shared processes, referral pathways, knowledge sharing, and coordination capacity help ensure continuity of care across providers.
